In person

To request your health records in person, visit our main campus on the lower level.

We will ask you to provide documents showing that you are the patient or the patient’s legally authorized representative.

Online

To request your health records online, follow these steps:

1. Fill out a Health Information Release form
2. Email the filled out form to np.him@hennepin.us

Fax

To request your health records via fax, follow these steps:

1. Fill out a Health Information Release form
2. Fax the filled out form to 612-302-4872
