Strategic Priorities
|
- Become a Hennepin County Enterprise Fund, to meet regulations for the Section 330 grant.
- Serve annually at least 70,000 patients in next five years.
- Implement physical remodeling of NorthPoint's facility.
- Create an urgent care and specialty clinics in remodeled clinic.
- Complete North Minneapolis facility site master plan, in partnership with Community Corrections and Human Services.
- Increase patient revenue by 13 percent from $10.8 million to $12.1 million.
- Continue organization-wide initiatives to improve efficiency and increase productivity and quality.
- Integrate mental health, primary care and human services.
- Implement a new practice management and billing system and eectronic medical record system.
- Implement the Gateway project and evaluate client outcomes.
- Develop the Innovation Center to create capacity for research/practice in North Minneapolis.
- Expand Community Health Worker Program.
